What you need to check
You must be a permanent resident of Queensland with a permanent disability. You must need a high level of support from a companion to take part in most community activities, for life. The card is free and lasts 5 years. A health professional fills in an assessment form, and you need a verified photo. It takes about 20 business days, then about 3 weeks for the card to arrive.
