What you need to check
You must live in the NT. A health professional must assess that you have a permanent or long-term disability, meaning longer than six months. The disability must stop you using public transport safely. You get a smartcard that pays half the fare and you pay the rest. If you use a wheelchair, the Lift Incentive Scheme also pays the driver $30 for each trip in an accessible vehicle. Fill in the application form, or call 08 8924 7229 to get one in the mail. Add two passport photos and proof that you live in the NT. A health professional completes Part B. Send it to GPO Box 2520, Darwin NT 0801 or cpv.admin@nt.gov.au.
This is a short summary. Other rules may apply, like income or where you live. Check the full rules with the agency that runs it.
