What you need to check
You must be a permanent Australian resident living in NSW and over school age. You must have a severe and permanent disability that stops you using public transport, such as mobility, vision, epilepsy, intellectual disability, speech or hearing. You will not qualify if treatment is expected to improve your condition. You cannot be in a similar scheme in another state. There is no income test, so you do not need a concession card. It also covers wheelchair-accessible taxis. An independent medical assessor reviews each application. You fill in Part A of the form, and your doctor or specialist does Parts B and C. Apply online or by post. For a paper form call 131 500.
This is a short summary. Other rules may apply, like income or where you live. Check the full rules with the agency that runs it.
