What you need to check
Coordinated Veterans’ Care helps you manage long-term (chronic) health conditions and stay out of hospital. Your GP writes a care plan with you, and a nurse at the practice keeps in touch. It is for Veteran Gold Card holders with a chronic condition. White Card holders can join if they have a chronic mental health condition that DVA has accepted. Your GP checks if you can join. You must live in Australia. You cannot join if you live in an aged care home (other than for respite), are in the Transition Care Program, or are expected to live less than 12 months. White Card holders may pay for some care in the plan that is not for their accepted conditions.
This is a short summary. Other rules may apply, like income or where you live. Check the full rules with the agency that runs it.
