## What you need to check

You must live in Tasmania and hold an eligible pension or concession card. There is an income test, so not every Health Care Card holder will get it. There are limits on how often you can apply. To apply, send the application form with your optometrist’s script for glasses. Call 1800 232 148 to ask about it, or for the list of optometrists who take part.
