## What you need to check

There is no cost for the card. You must live in Tasmania and be a permanent resident of Australia. You must have a lifelong and permanent disability. You must also need a high level of support from a carer to go to community events. You do not need a concession card. Apply with the form and a passport-sized photo that is less than 6 months old. A health professional or service provider must sign the form and photo. Post it to Companion Card, Service Tasmania, GPO Box 123, Hobart TAS 7001, or hand it in at a service centre. Phone 1800 009 501 for a form. It can take several weeks.

This is a short summary. Other rules may apply, like income or where you live. Check the full rules with the agency that runs it.
